Background

The patient presented with extensive carious involvement in the anterior quadrant, compromised crown height, and reduced structural integrity, affecting both function and aesthetics

Treatment Plan

A phased multidisciplinary approach was planned:

  1. Conservative caries management

  2. Periodontal surgical intervention (surgical crown lengthening)

  3. Structural reinforcement (post + core)

  4. Definitive prosthetic rehabilitation

Caries Management

  • Selective caries removal was performed to preserve sound dentin

  • Aimed to maintain tooth ferrule and avoid unnecessary structural loss

Surgical Crown Lengthening

Due to insufficient clinical crown height and subgingival caries margins, surgical crown lengthening was performed to:

  • Establish a sound tooth structure above the gingival margin

  • Respect biological width

  • Improve restorability and long-term prognosis

Post and Core Restoration

  • A post and core was placed in the indicated tooth to reinforce the remaining structure

  • Provided adequate retention and resistance form for the final prosthesis

Definitive Restoration

An anterior fixed bridge was fabricated to restore:

  • Aesthetics

  • Function

  • Proper anterior guidance
